HIPAA/Corporate Compliance Concern Form
The Community Emergency Medical Services Corporate Compliance and Privacy Plan and Standards of Conduct (as reflected in the document of the Botsford Health Care Continuum).

As modified in 2002, the Plan includes specific information about our expectations for an ethical workplace. The Standards of Conduct serve as a guide for decisions we make daily regarding caring for our patients and in conducting other Community EMS business. Also covered in the Standards are guidelines to follow if you suspect a potential compliance or patient privacy concern within CEMS. Please refer these concerns to your supervisor, administrator, Connie Williams at Botsford, the Continuum’s Corporate Compliance and Privacy officer (248-871-8660), or Greg Chittle, our Quality Assurance Peer Review Coordinator at 248-356-3900 ext. 6073. Anonymous reporting of such concerns can be done via the Direct Helpline at 877-471-2422, which is not connected to caller ID, or by submitting a report through our anonymous web mail by clicking the link below.
